DEPARTMENT:

Outpatient Clinic

 

LINES OF AUTHORITY:

No supervisory responsibilities for this position.

This position is supervised by:  Director of Outpatient Services

Assistant Director of Outpatient Services

 

POSITION SUMMARY:

The Case Manager’s main purpose is to ensure their client caseload is provided with the necessary services in a coordinated and effective manner.

 

PRIMARY FUNCTIONS/RESPONSIBILITIES:

-  Complete required hours of training/continuing education (48 hours in the first year of employment, 24 hours in subsequent years)

-  Complete all company required training as outlined in the Staff Development Plan

-  Practice ethical behavior for the profession at all times

-  Participate in supervision as required (50 hours/year if not certified, 12 hours per year if certified)

-  Maintain a minimum average of 50% productivity

-  Coordinate care with families/significant others, payer sources, primary care providers, Child Protective Services, Juvenile/Adult Probation, schools, foster-care placements, etc.

-  Ensure that all documentation supports the diagnosis and the services provided

-  Develop individualized treatment plans and update according to program timeframes

-  Complete progress notes at the end of each service and submit within 24 hours

-  Complete all required paperwork updates in client charts according to program timeframes

-  Complete discharge summaries on discharged clients according to program timeframes

-  Complete staff activity logs and submit daily

-  Accurately complete staff activity logs

-  Complete Medical Chart Review forms monthly according to program requirements

-  Communicate effectively with peers, supervisors and community agencies

-  Use work time effectively

-  Work cooperatively with supervisor/s

-  Work cooperatively with other professionals

 

PRIMARY CONTACTS-INTERNAL & PUBLIC:

* Frequent interaction with clients and their support system.

* Interact with clinical team and support staff.

* Interact with other SMMHC, Inc. programs for coordination of client services.

* Interact with mental health entities, community mental health programs, hospitals, AHCCCS, Social Security, DES, Vocational Rehabilitation, and other community resources as needed in effort to provide client with necessary services.

 

POSITION REQUISITES:

 

EDUCATION/ EXPERIENCE:

High School or GED with at least four (4) years full-time work experience in behavioral health; OR High School or GED plus 18 credit hours post-High School education in field related to behavioral health completed nor more than four (4) years before the date of application; OR Bachelor's degree in a field related to behavioral health plus two (2) years full-time work experience in behavioral health; OR Bachelor’s degree in behavioral health. 

KNOWLEDGE/SKILLS/ABILITIES:

* Must have extensive knowledge and background of all mental health populations.

* Must be knowledgeable of benefit and entitlements process and community resources available.

* Must be familiar with current psychotropic medications and side effects.

* Must be experienced and trained in crisis intervention.

* Must have good oral and written communication skills.

* Must have specialized knowledge and skills relative to growth and development of children and   adolescents.

* Must be able to apply an appropriate range of treatment interventions to children and   adolescents.

 

PHYSICAL HEALTH REQUIREMENTS:

Must be in good health.

Series of Hepatitis B shots or declination of same at hire

Proof of negative TB test

 

OTHER CERTIFICATIONS/LICENSES NOT INCLUDED ABOVE:

Certification in CPR and First Aid

Completion of CPI one time

Possession of a current and valid driver's license and proof of insurance.

Fingerprint clearance.

Certification of AZ Board of Behavioral Health Examiners preferred.
