Social Services Designee, FT
Qualifications:

• BSW or related human services field
• Minimum 1-year work experience in the area of social work
• Excellent communication skills – must possess proficiency in English and Japanese speaking is required

Duties:
• Assesses residents upon admission and implements strategies to assist the residents, families, and others significantly involved with the resident to successfully transition to facility placement
• Monitors the resident’s emotional and social needs and works with the interdisciplinary team to develop interventions to maintain or improve resident outcomes
• Serves as the communication liaison between contracted medically related ancillary service providers and the residents, families, responsible parties, nursing, and administrative staff
• Assists in coordinating transportation and medically related ancillary services
• Provides resource information and assists residents and families in areas including, but not limited to, Social Security, Medicare and Medi-Cal programs, hospice, home-health, advance directives, resident rights, abuse, grievance, safeguarding valuables, psychotropic medications and counseling, medically-related ancillary services, community and legal resources and other health care facilities
• Assists residents and families to develop and implement advance directives
