The Infant & Toddler Connection of Virginia is Virginia’s system of early intervention supports and services for infants and toddlers from birth through age two who are not developing as expected or who have a medical condition that can delay normal development, and their families. Early intervention services are designed to meet the full range of developmental needs of each child, and the needs of their families related to their child’s development.
Service Coordinators must have strong interpersonal skills enabling effective communication with families, service providers, and other professionals. Incumbent must also work effectively as a member of a multidisciplinary team and in the families’ natural environment.
SERVICE COORDINATORS PROVIDE THE FOLLOWING SERVICES:Assessment of family/consumer needs; Facilitation and coordination of the multi-disciplinary assessment; Development of the Individualized Family Services Plan (monitoring and implementation);Data compliance with local, state, and federal mandates; Meeting all requirements for State Planning Option service coordination.
EDUCATION, WORK EXPERIENCE, LICENSURE, CERTIFICATION:Bachelor’s degree, plus a minimum two (2) years experience with young children (birth to 36 months);Knowledge of typical early childhood development;Knowledge of family supports and services required by infants and toddlers with developmental delays or diagnosed disabling conditions, including atypical development; Awareness of available programs and services, as well as, basic eligibility criteria for these programs;Knowledge of PART C and MEDICAID State Option Plan (preferred).
Pay Band 4; salary commensurate with qualifications and experience. Commonwealth of Virginia benefits. Successful fingerprint criminal background check is required. Potential risk of exposure to infectious diseases in homes.
