Telecare LA Adult 7 program will open November 1st in the city of Norwalk.  Telecare is seeking professional staff with experience in community-based mental health services. Staff will provide services to adults with serious persistent mental illness.  Our member’s population consists of adults who may be homeless or have forensic involvement. 

 

A Personal Service Coordinator II provides direct client community support and case management services to assigned members using Psychiatric Rehabilitation and Recovery principles.  Provides education and support to members and their families.  Collaborates with other services and agencies such as the Conservator’s Office, Public Defender’s Office, Child Protective Services, Probation Department and County Mental Health Services.  Completes assessments and determines the appropriates level of services for new members.

 

The successful candidate will be a high school Graduation or G.E.D. equivalent and four years of direct services in Mental Health; or Associate Arts degree and two years of direct services in Mental Health; or BS degree in Social Sciences and one year experience in Mental Health Services are required.  Experience will have been in a mental health setting, out patient clinic, or community mental health service preferred.  Experience in providing intensive case management, intensive community supports using psychiatric rehabilitation concepts is highly desirable. Sensitivity to multi-cultural populations and issues is required. Necessary skills include: the ability to work effectively as a member of a multidisciplinary team, a thorough understanding of client rights, an understanding the conservatorship process, and knowledge of basic medical and psychiatric terminology.
