As a Medical Social Worker with Gentiva, you will:
•Instruct, treat, observe and evaluate patients exhibiting significant social and emotional problems affecting their health status.
•Assist the physician and other health team members in understanding the significant social and emotional factors related to the patient’s health problems.
•Participate in the development and periodic re-evaluation of the physician’s Plan of Treatment for patients needing social work services.
•Utilize special skills and judgment to resolve social and emotional responses connected with the illness.
•Observe, record, and report changes in patients’ condition and response to treatment to the Clinical Manager and physician. Participate in the discharge planning process.
•Instruct/counsel patients and families in treating and coping with social and emotional response connected with illnesses and provide ongoing assessment of patient and family needs and responses to teaching.
Qualifications
•Master’s Degree from a school of social work accredited by the Council on Social Work Education
•Six months social work experience in a healthcare setting. If working in a Medicaid/Medicare certified home health agency, the Medical Social Worker must have at least one year experience in a healthcare setting
•Current licensure/certification in the state(s) of practice if required
•Good organizational and communications skills
•Ability to function with minimal supervision
