Licensed Clinical Social Worker

Our Nassawadox Facility currently has an opportunity available for a Licensed Clinical Social Worker!The Social Worker will provide psychosocial services to all patients treated by the facility including in-center and home dialysis patients (if applicable).  In this position, you will evaluate each patient at admission and formulate appropriate treatment plan, provide ongoing casework and advocacy services for the patient to ameliorate medically related social or emotional problems, inform and clarify for patients and their families the facility policies and resources.  The Social Worker re-evaluates at appropriate intervals those patients not seen on a regular basis as well as maintaining records in accordance with the policies of the Social Service Department and the facility in order to comply with E.S.R.D. regulations.

EXPERIENCE AND REQUIRED SKILLS:



Experience with renal patients or other physically ill population preferred.

 

EDUCATION:

 

Master’s Degree from an accredited school of social work.  Certification: LCSW required.
