Habilitative Specialist/Case Manager

Job Summary:

Directs and coordinates the delivery of habilitative services in compliance with regulatory requirements.  Responsibilities include training staff in behavior modification techniques, developing new programs and maintaining records.

 

Essential Job Functions:

1. Direct and coordinate the delivery of habilitative services in compliance with regulatory requirements.

2. Train staff in behavior modification and/or intervention techniques.

3. Manage development of individual-centered programs.

4. Monitor the quality of services delivered to individuals served.

5. Develop new policies, procedures, and methodologies for delivering habilitative services.

6. Be on-call to provide assistance and guidance to staff when problems arise.

7. Document individual progress and maintain records or persons served.

 

Knowledge, Skills, Abilities:

1. Knowledge of company policies and procedures.

2. Knowledge of habilitative services for individuals with developmental disabilities.

3. Problem-solving skills.

4. Planning and organizational skills.

5. Ability to develop innovative approaches to implementing habilitative services.

6. Ability to read and understand policies and procedures.

7. Ability to read and understand regulatory requirements related to habilitative services.

8. Ability to maintain confidentiality.

9. Ability to work in an on-call capacity as required.

 

Education and Experience:

Bachelor's Degree in Psychology, Special Education or related field.  Minimum of one year of experience working with individuals with developmental disabilities required.  May require QMRP certification and experience in an ICFMR.
