Case Manager

 

 

Purpose of Position

Work in a multi-disciplinary team to provide services for persistently mentally ill adults in the Glendale area.  Assist clients in gaining necessary services (medication management, housing, Medi-Cal etc.).

Qualifications:

·  BA/BS in Social Science OR 1 year relevant work experience

·  MUST BE BILINGUAL-SPANISH

·  Efficient organization skills to manage high activity level

·  Knowledge and sensitivity to cultural differences

·  Excellent written and oral communication skills

·  Initiative, flexibility, creativity and enthusiasm

·  Detail oriented

·  Affinity for working with adult individuals with mental illness

·  Patience and persistence

·  Ability to manage large amounts of paperwork

 

Functions of the Case Manager:

·  Collaborate with and translate for psychologists, psychiatrists, nurses, other support staff and community agencies to provide the clients with comprehensive treatment

·  Meet with the client outside of the clinic as needed

·  Identify and resolve client crisis

·  Attend weekly meetings and trainings

·  Interface with community agents (e.g.- Department of Public Social Services, Jail, Landlords etc.) on the clients behalf

·  Provide feedback to treatment team on case management issues

·  Complete Department of Mental Health Paperwork

·  Maintain confidentiality

·  Keep Program Coordinator and Manager well informed regarding activities, pending issues and potential problems

 

Physical Requirements:

·  Able to work at a computer for hours at a time

·  Able to perform the following tasks during the work day:  reading, writing, speaking, listening, typing, sitting, standing, walking and driving a car
