Wound Care RN

JOB SUMMARY: The Wound Ostomy Continence Nurse is a licensed Registered Nurse who provides comprehensive nursing, consultative, and educational services to individuals, families and groups in need of wound, ostomy, and/or continence care.  The WOCN must be proficient in the nursing process, demonstrate clinical expertise and function with a high degree of autonomy, accountability, and collaboration.

 

MINIMAL TRAINING AND QUALIFICATIONS:

 

§ Must have a current license to practice as a Registered Nurse in the State of Alabama and must maintain licensure throughout employment with Agency. 

§ Graduation from a Baccalaureate Degree Program accredited by the NLN preferred. 

§ Completion of education from a reputable Wound Ostomy Continence School and certification as a WOCN.  Must maintain certification. 

§ Minimum of one year in a hospital setting and at least one year in home health. 

§ Minimum of 1-2 years experience as a WOCN preferred. 

§ Must have and maintain a reliable source of transportation 

§ Must possess a current Alabama driver’s license and proof of automobile liability insurance.

 

ESSENTIAL JOB FUNCTIONS:

 

§ Demonstrates an appropriate nursing knowledge base with emphasis on wound, ostomy, and continence care.

§ Acts as a consultant to staff and other professional health care community case conferencing, patient visits, through inservices/education offerings and in orientation of new staff.

§ Demonstrates ability to function in a self-directed role.

§ Utilizes the nursing process in delivery of patient care.

§ Possesses knowledge of Medicare and other 3rd party reimbursement criteria appropriate to tenure with the Agency.

§ Fulfills documentation requirements of appropriate Agency forms and reports.

§ Participates in the case management process of patients to assure that patient outcomes are met utilizing resources appropriately.

§ Schedules patient visits in accordance with plan of care as directed by the physician.

§ Collaborates with Administrator for product recommendations in areas of expertise.

§ Adheres to Agency policies and procedures in delivery of patient care.

§ Performs patient care activities in accordance with Mid South Home Health Agency and Community Health Nursing Standards.

§ Reflects adherence to Medicare and other 3rd party reimbursement criteria in delivery of patient care and documentation.

§ Completes all documentation thoroughly, accurately, and in a timely manner according to Mid South policies and procedures.  Submits daily.

§ Utilizes appropriate infection control measures in delivery of patient care.

§ Patient care delivery, case conferences, consultative services, and inservices are reflective of appropriate knowledge of wound, ostomy, and continence care.

§ Meets productivity standards as established by the Agency.

§ Appropriately manages referrals for consultation in a timely manner.  Prepares written report for RN case manager.

§ Serves on committees both inside and outside the Agency, which are beneficial to the operation of the Agency program.

§ Checks with RN Team Leader in Am for changes and messages for the day.  Checks out with RN Team Leader at the end of the day with rreport on patients seen and inability to complete any assignment.

§ Participated in Administrative call, weekend and holiday work as requested.

§ Becomes trained in and utilizes computerized medical record system on all documentation. 

§ Is responsible to maintain all equipment in good working orderl

§ Maintains patient confidentiality.
