Responsible for the direct supervision of personnel in a given patient care area as well as providing direct patient care daily. The Care Team Supervisor LVN manages daily departmental operations and functions in a given care area.

Performs simple to moderately complex nursing procedures within the scope of practice of a Licensed Vocational Nurse.
Supervises personnel in a given patient care area.
Providies direct patient care.
Obtains, interprets, records and reports vital signs, (temperature, pulse, respiration and blood pressure) height, weight and other pertinent patient information to the provider.
Prepares patients for examinations and other procedures.
Implements a plan of care by performing nursing interventions and assisting with procedures.
Instructs patients on procedural preparation.
Performs various patient tests and procedures within the scope of practice of the LVN nurse.
Administers medications and other therapeutic modalities within the scope of practice of a LVN.
Collects, labels and transports various specimens to appropriate destination.
Maintains clinical standards outlined in the clinical operations policy and procedure manual; identifies deficiencies and makes recommendations for change.
Documents and maintains records of medical and nursing procedures.
Coordinates with providers and ancillary departments to plan daily work assignments according to staff abilities, patient schedules, patient flow, provider schedules and procedures.
Ensures adequate levels of supplies are maintained.
Ensures that all equipment is maintained.
Performs telephone screening under the direction of a provider.
Manages ordering of all supplies.
Notifies pharmacies of new prescriptions and authorizations of refills as ordered by the provider.
Participates in educational programs to enhance nursing knowledge.
Adheres to Universal Precautions at all times.
Adheres to safety policies and procedures at all times.
Assists the Site Administrator / Manager in ensuring compliance with certification, competencies, and compliance measures.
Evaluates MA competencies.
Participates in Care Team member hiring and termination.
Backs up CTM I and CTM II as needed.
Coordinates staff coverage.
Ensures proper staffing/scheduling CTM rotations.
Performs evaluations.
Responds to emergencies within the site.
Identifies and participates in quality improvement projects.
Performs other duties as assigned.

Current licensure in the State of California.
A high school diploma or G.E.D.
One or two years of post-high school education or degree from a two-year college.
Up to and including 1 year of experience.
Over 1 year and up to and including 3 years of experience.
Graduate of an accredited Licensed Vocational Nurse program.
Basic Life Support for Health Care Providers.
