Pediatric home health nurses for shift care
Job Purpose:
Enhances patient health by providing one on one nursing care to the pediatric patient in the home setting.
Duties:
* Provides all of the patient care to include: assessments, ADL's, PO, GT or NG tube feedings, respiratory care which may include nebulizer, Bi-Pap, tracheostomy and ventilator care, administers medications and monitors patient for adverse reactions, assists family on patient outings to MD appointments and other appointments as necessary.
* Functions as an important part of the healthcare team. Provides relevant patient information to the Agency RN Case Managers, Service Coordinators, Physicians, Family caregivers and other team members as appropriate.
* Educates patient caregivers as needed on diagnosis, medications, treatments, and equipment. Attends agency in-services to remain up to date on advancements in medical technology.
* Documents all nursing care provided during the shift according to agency and regulatory policies. Maintains patient confidentiality. Maintains professional nursing boundaries.
* Serves and protects the home care agency by adhering to professional standards, home care policies and procedures, federal, state, and local requirements, and professional and licensing standards.
