We’re looking for a self-starter that demonstrates the ability to work well with little direction when needed, as well as part of a team. A high degree of patience and understanding are key, as you will be assisting in coordinating care for end-of-life patients. This is a particularly good opportunity for an LPN, but also open to an experienced customer service or administrative professional interested in growing in the healthcare field. The primary responsibilities of this position include:
Communicating with patients, facility staff and internal team members by phone
Answering calls related to the coordination of care
Assisting with arranging CNA visits or ordering medical equipment
Preparing orders for “start of care” to schedule initial RN consultations
Following up on referrals for patients regarding the RN consultation
Setting up cases in the system
Managing daily documentation
Entering patient notes and chart information accurately and in a timely fashion
Conducting basic computer-based research for provider and insurance information Interacting with team assistants and marketing staff to maintain healthy relationships with the facilities our patients our utilizing



Customer service and administrative experience is required. LPN experience is strongly preferred. Any experience with hospice and telephonic customer service would be helpful. A bachelor’s degree is preferred. MS Office and email skills are needed.
